Instructions:
Click on the following link to download credentialing forms in Microsoft Word and Excel formats. All forms will download as one "self extracting" file called "CREDETIALING.exe". Upon seeing the download window, select the "save" option, to save this file on your computer. Double click on the saved file to automatically extract all the forms.
Please complete the APPLICATION form and the REQUEST FOR PRIVILEGES form (for your specialty) and submit them along with a current signed copy of the STANDARD ILLINOIS CREDENTIALING FORM.
The first page of the APPLICATION form has a check list for all the documents which are needed.
